Insurance and Physical Therapy Benefits
We are in network for Blue Cross Blue Shield of Illinois PPO plans. For out of network benefits, we can provide you a superbill that you can submit to your insurance for reimbursement.
Self Pay and out of network Physical Therapy Rates
Physical Therapy 60 minute Initial Evaluation is $200
Physical Therapy follow up visits are $175
For patients who are uninsured or choose to self-pay, we provide a Good Faith Estimate of expected charges before treatment and a detailed superbill after each visit that may be submitted to insurance for potential out-of-network reimbursement.
Cancellation Policy
We require 24 hour notice for cancellations. A fee of $50 will be charged for no shows or late cancelations.
Prescriptions
Illinois is a direct access state and a prescription to start PT is not required.
